Medicaid LTC (Statewide Medicaid Managed Care – Long-Term Care) is the most common waiver for seniors in Florida and pays for adult day care, transportation, and meals through a managed plan like Sunshine or Simply. Most Hialeah families qualify for LTC.

From the first ADRC call to the first day of care, usually 2 to 6 weeks. The CARES evaluation takes 7 to 14 days; plan enrollment another 10 to 21 days. We keep you updated.

CARES (Comprehensive Assessment and Review for Long-Term Care Services) is a free visit from a Florida state assessor who confirms clinical need. It happens at home or at the center. Having a bilingual family member present helps — we coach you on what to expect.

Does it cover transportation to the center?

Yes. Medicaid LTC covers door-to-door transportation within a 15-mile radius of either of our two locations. Zero gas costs for the family.

My loved one already has Medicaid but not LTC — can we add it?

Accepted Answer

Yes. If your loved one already has regular Medicaid or a managed care plan, we can request the addition of the LTC (Long-Term Care) benefit. The process is faster — usually 3 to 4 weeks — because financial eligibility is already validated.
