How we handle the application for you — 4 steps

    Our coordinator asks about age, income, and medical conditions. We quickly know if it's likely they qualify for CDC+ or LTC.

  2. 2. Gather documents and open an ACCESS Florida account

    ID, Social Security, proof of income, medical record — all in one folder. We open the account at myflfamilies.com/access with you.

  3. 3. Coordinate the CARES assessment or connect with ADRC

    For LTC, we refer to the Aging & Disability Resource Center (305-670-4357). For CDC+, we schedule the Department of Elder Affairs assessment.

  4. 4. Start care in 2-12 weeks

    Once approved, we coordinate free transportation, meals, and care plan. No copay for most families.

What does this all cost?

The application is free. The paperwork help is free. When your loved one starts, Medicaid pays the center directly — no copay for most families. If the application is denied, we charge you nothing. We do it because our community deserves this resource.

Frequently asked questions about Medicaid

What's the difference between 'regular' Medicaid and Medicaid LTC?

Regular Medicaid covers basic medical services (doctor visits, hospital, medications). Medicaid LTC (Long-Term Care) is an additional benefit that covers day care, transportation, and meals for seniors needing supervision. Many families have regular Medicaid but don't know they can add LTC.

Can my family member have Medicare and Medicaid at the same time?

Yes. That's called 'dual eligible.' It's very common among Hispanic seniors in Hialeah. Medicare covers medical care; Medicaid LTC or CDC+ covers day care.

Do you accept all 5 major Medicaid LTC plans?

Yes. At both locations (Hialeah Gardens and Hialeah) we are in-network with Sunshine Health LTC, Simply Healthcare LTC, Humana LTC, Aetna Better Health LTC, and Molina Healthcare LTC.

How long does LTC approval take vs. CDC+?

LTC: usually 2-6 weeks from the first ADRC call to the first day of care. CDC+: 6-12 weeks because it includes the CARES assessment and DCF processing.

My mom has a small pension from Cuba or another country — does it count as income?

Yes, any foreign pension converted to dollars counts as monthly income. We declare it on the application. Most modest pensions don't disqualify, but they must be reported to avoid fraud issues.

What if my family member is not yet a citizen?

Permanent residents (Green Card) with 5+ years generally qualify. Refugees, asylees, and parolees too — but the rule changes case by case. Call us and we review together with no commitment.

Does Medicaid cover transportation to the center?

Yes. Both CDC+ and LTC cover door-to-door transportation within 15 miles of the center. Zero gas or Uber expense for the family.

What if my application is denied?

You have 90 days to appeal. Most denials due to documentation issues are won on appeal if the papers are correct. We accompany you at no additional cost.

My family member already has Medicaid but not LTC — can we switch quickly?

Yes. If they already have 'regular' Medicaid, requesting the LTC addition is faster — usually 3-4 weeks — because financial eligibility is already validated.

Do you have an exact eligibility calculator?
